"The Xavier University Telehealth Center (XUTC) aims to provide medication related care to patients in the Greater New Orleans area and beyond. The Center will be student run, offering them the opportunity to have hands-on experience with Medication Therapy Management (MTM) and Chronic Care Management (CCM) services. The XUTC will be conveniently located in the College of Pharmacy and will be facilitated by our MTM trained faculty.”

What is Telehealth

Telehealth is the use of digital information and communication technologies, such as computers and mobile devices, to access health care services remotely and provide patient care. Pharmacists and pharmacy students are able to provide this level of care through a variety of different avenues.

Current Services Offered

• Medication Therapy Management - a review of a patient's entire medication profile to identify and resolve medication related problems and the development of  plans in coordination with the patient and provider.
• Chronic Care Management - detailed patient education and consultation specifically related to diabetes and hypertension.
• Diabetes Management Support for Rural Clinics - providing diabetes support from a pharmacy standpoint (in collaboration with other diabetes health care specialists) to rural clinics.
