You have three options for completing the application:

• Option 1) Fill out the form on the STEM Saturday Workshop Series brochure and then send it to us at the address below).
• Option 2) Download the entire application as a PDF file (you will need to print the file, fill out the form, and then send it to us at the address below).

• Option 3) Request a printed application. Send email to xueng@xula.edu or a letter with your name and address, and we'll send you an application.

Applications may be faxed to (504) 520-7966 in order to meet the deadline, but because faxes are often difficult to read, copies should also be mailed to the address below.
