SUBMISSION OF APPLICATION FOR FALL 2010 IS CLOSED

Please read all the information below before submitting your application for admission.

The College of Pharmacy application period opened August 1 and closed October 15 at 12:00 Midnight Central Standard Time.   To view the status of your current application, use your Login ID and PIN on the On-Line application “Login Page”.

DUE BY DECEMBER 1, 2009 (except PCAT scores) - Supporting documents, including the “Admissions Application Certification & Signature Page” and $25.00 Application Fee, For completion of the application process

Please read all the information below FOR SUBMISSION OF SUPPORTING DOCUMENTS

All supporting documents (except PCAT scores) are due at the close of the business day December 1.   When the deadline date falls on a Saturday or Sunday, all materials must be received by the end of the business day on the following Monday.  You are encouraged to submit all supporting documents early and monitor your application checklist online.  Documents received after the deadline date are considered LATE and will not be reviewed.   Documents postmarked on the deadline date will not be considered for completion of the application process.  If you have any questions or concerns about this process, please e-mail us at applyxucop@xula.edu.Supporting documents include:

1. Recommendation Forms (3 required) 

Arrange to have three separate “Faculty Recommendation Forms” sent directly to the Xavier University Office of Admissions by the recommenders.  The recommendations must be from physical/life science or mathematics instructors who have taught the applicant at the college level.  These recommendations must be submitted on the “Faculty Recommendation Form”.   Letters are not acceptable in place of the approved form.

As an option, one of the three recommendations may be from someone, preferably a health professional, under whose supervision the applicant has worked.  If you choose to use a supervisor as a recommender, please use the designated “Employer/Supervisor Recommendation Form”.

Unacceptable recommendations will render the application incomplete; such applications will not be given further consideration for admission.  The following recommendations are not acceptable:

  • recommendations mailed to the University by the applicant
  • recommendations from relatives or friends
  • faxed or hand-delivered recommendations by the student
  • faxed recommendations by the recommender

2.  Supplemental Information Form (Pharmacy Instruction Form)

This form must be completed and submitted with the application “Admissions Application Certification & Signature Page”.

3Pre-Pharmacy Requirements Form

The cumulative GPA is calculated on course requirements specified in the pre-pharmacy curriculum.  A separate GPA is calculated on required science and math courses.   Grades for all coursework attempted, including D’s and F’s are included in the calculation.   This form must be completed and submitted with the on-line application “Signature Page”.

4. Pharmacy College Admission Test (PCAT) scores. 

Effective Fall 2009, the PCAT is a new requirement for the College of Pharmacy application process.  All of the dates below meet the requirement for submission of test results.  PCAT registration and test date information can be located on the web at: http://tpc-etesting.com/pcat/.
The available test dates are: 
August 27, 2009
October 17, 2009
January 23, 2010

5. Official transcripts from each college/university attended including:

  • Official transcripts of credits earned through cross-enrollment or summer sessions enrollment.  These transcripts must be requested from the Registrar’s Office of the institution granting the credit.  Grades/credits posted on other transcripts are not acceptable.
  • Verification of Enrollment - If you are enrolled for the first time at an institution other than Xavier University and the institution is unable to generate an “In-Progress” transcript, verification of current enrollment must be sent from the Registrar’s Office of that institution.
  • Concurrent and/or dual enrollment coursework completed while in high school.
  • International Baccalaureate (IB)
  • Advanced Placement grade report from the College Board at (609) 771-7300 or (888) 225-5427 (toll-free in the United States and Canada). 
  • Foreign College/University: An official detailed course-by-course transcript evaluation for each foreign college/university attended.   This report must be sent directly from a foreign transcript evaluation service. Copies of reports or courses listed on other transcripts are not acceptable.  The agency recommended  is:

All transcripts must be mailed in a sealed institutional envelope directly to the Office of Admissions from each college or university attended.  Transcripts that are hand-carried or mailed by the student, even if in a sealed envelope, are not official and will not be accepted for purposes of admission.  Applicants who fail to report all institutions attended on the on-line application, including summer sessions, forfeit their eligibility for admission.   

6.  A minimum 250-word Statement of Interest detailing the applicant’s motivation and professional career goals for pharmacy.

7.  Non-citizens of the United States must submit a Certificate of Financial Support and one of the following:

  • a legible photocopy of the applicant’s Permanent Resident Card (Green Card)

    OR
  • a notarized document from a financial institution verifying availability of funds for each sponsor(s)

All documents must be received by the stated deadline to complete the application process.  The deadlines are strictly enforced.  No exceptions are made.  The University is not responsible for delays in mail delivery.   Do not send documents to the College of Pharmacy or Registrar’s Office.   All documents must be mailed to the following address

Admissions Process

Selecting a candidate for admission to the College of Pharmacy involves many important factors.  Final selection of the class will be determined by the overall strength of the applicant pool.

Interview Process

After a preliminary assessment of all completed application packets including all supporting documents, qualified applicants are invited for a personal interview with the members of the College of Pharmacy faculty.   The purpose of the interview is to assess oral communication skills, writing skills, motivation for pharmacy, critical thinking/decision-making abilities, knowledge of the profession of pharmacy and issues affecting health care, and leadership abilities.   If an interview is granted, the College of Pharmacy will contact the applicant by telephone to schedule an appointment for the interview.  Interviews are not conducted by telephone. 

Final Selection Process

Applicants will be notified by mail of the final admission decision.  If admitted, applicants must submit a non-refundable $500.00 deposit. This deposit must be received by the stated deadline along with a signed “Statement of Course Completion”.  Admission may be rescinded if the applicant fails to meet all admissions standards including: 1) failing to submit final official transcripts of Fall and/or Spring grades; 2) having course deficiencies that exceed the seven (7) credit hours needed to complete the pre-pharmacy requirements; 3) earning a grade of C- or below in required courses or 4) having a cumulative or Mathematics/Science grade point average that falls below 2.75.   Applicants should also have official transcripts of all coursework taken during the Fall sent by January 31st and during the Spring sent to the Office of Admissions by June 15th.  Transcripts from summer work must be received by August 1.

Withdrawal of Application

The deadline to withdraw an application to the College of Pharmacy is January 15th.   Applicants must submit a brief statement to the Admissions Office requesting withdrawal of their College of Pharmacy application. Requests to withdraw which are received after this date will not be considered.

Incomplete applications are not considered.

An application is incomplete if the candidate does not:

  • submit the application on time
  • complete all information on the application
  • submit all official transcripts
  • pay the application fee on time
  • have all “Recommendation Forms” submitted 
  • use appropriate recommenders
  • take correct required courses
  • submit  PCAT scores
