College of Pharmacy Readmission Procedure

The College of Pharmacy Academic Standing Committee will review requests for readmission by former pharmacy students who were previously enrolled in the COP with either "Good Academic Standing" or "Dismissed" academic standing status during the last semester attended. 

A student who left the university in "Good Academic Standing"  and wishes to return should submit an Application for Readmission directly to the Assistant Dean for Professionalism at the following address:

A student who has not been enrolled due to an "Academic Dismissal" or "Suspension"  must complete the following: 

1)  Application for Readmission

 2) Request for Readmission Form

3) Probationary Action Plan. If student's request for readmission is approved, the Probationary Action Plan will be due prior to the first day of class during the semester in which the student re-enrolls. This  document should be submitted to the Office of Student Affairs in the College of Pharmacy.

A student who was not in good academic standing (cumulative G.P.A. below 2.0) when he or she left the College of Pharmacy must schedule an appointment with the Counseling Center after an application for readmission has been submitted.   

If a request for readmission is granted, the student will receive a letter from the COP Dean listing all required conditions that must be met before and after re-enrollment into the COP.  Note:  All students who are readmitted to the COP after a Dismissal or Suspension, will be placed "On Probation" the semester of re-enrollment and therfore required to participate in the COP's Academic Enrichment Program (AEP).

If a student is denied readmission by the College of Pharmacy Academic Standing Committee, the student has a right to a written appeal of the decision to the University Academic Standing Committee. This appeal (University Academic Appeal Form) must be received in the Registrar’s Office by July 1.
